I am an "out of plan provider." I DON'T ACCEPT ANY INSURANCE -- EVER -- NOT EVER. If you have a PPO (Preferred provider organization), it is possible that your PPO medical insurance company will provide partial payment for an out-of-network provider.
It is your responsibility to determine if your insurance company will provide any payment for your psychotherapy session.
I also accept FSA (the medical FSA, flexible spending account, flexible spending arrangement, or the "cafeteria plan") methods of payment. I may attempt a prior authorization for your medication prescription.
If prior authorization (PA) exceeds a reasonable time, I will be charging $200 per hour in 15 minute increaments. We can also attempt the insurance prior authorizaiton during your session time at no additional fee.
Reports, Fitness for Duty, Legal Reports, Psychiatric Reports, Disability Reports.
I provide legal psychiatric reports for many different purposes. (Also called forensic psychiatry)
In general, the report will require in person meetings for interview, assessment, information gathering, review of documents and psychiatric testing.
I will perform psychological testing and urine testing as specific needs arise.
The reports vary in length from one page to 20 pages. The cost is determined by the time we meet and the number of pages. The number of pages is determined by the requirements of the report.
In general, the cost is about $100 - 200 per page plus the time for face to face meeting.
As an example, a typical four page fitness for duty report, including interview is approximately $800.
The cost increases for time contraints, fast turn around or contraints in my schedule.
I offer explanation, clarification and defense in a deposition or court setting at an additional fee.

Scheduling of sessions can be quite flexible, with consideration of the patient's travel, work and other circumstances. Once an in-person therapeutic relationship has been established with a California resident, sessions can be conducted by phone or videoconferencing. In certain circumstances, telephone and/or videoconferencing sessions have proven to be an effective way to maintain the continuity of psychotherapy. Week-end and evening appointment are available.
Cash, check, paypal, Venmo, credit card, debit card or flexible spending account (FSA) is accepted for payment.
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
